IMPORTANT: Your Health Coverage Tax Documents (Form 1095-C)

Form 1095-C Availability Notice

In accordance with the Paperwork Burden Reduction Act, Catoosa County Public Schools is no longer required to automatically mail a paper copy or electonically deliver Form 1095-C to all employees.  Form 1095-C provides information about the health insurance coverage offered to you and is not required to be filed with your tax return; however, you may wish to keep it for your records.

  • You have the right to receive a copy of your Form 1095-C upon request. You may request your form using any of the following methods:

  • When making your request, please specify your preferred delivery method:

    1. Electronic Delivery (Recommended): To receive your form via email, please provide written consent in your request. 
    2. Paper Delivery:  If no preference is stated, a paper copy will be sent in interoffice mail to active employees or mailed to your address on file to inactive employees.

    Timeline: CCPS will provide your requested form within 30 days of receiving your request.

    If you request electronic delivery, you will receive an email to your county email from Amy Perry with Subject: Electronic Consent for Form 1095-C. You will need to follow the directions in the email before the Form 1095-C will be sent to you electronically.
